HornsteinTrust.com provides an online questionnaire and helpful information towards the goal of assisting you in the preparation of an estate plan, including documents such as a Trust, Will, Powers of Attorney for Management of Property, and Powers of Attorney for Health Care, Physicians Directives, as well as documents to fund your Trust, such as deeds to your real property, and Assignments of Interest of your business to your Trust, and Declarations of Intent to declare your intention to fund your Trust. While the site provides general information and offers common provisions often included in such documents, there is no legal advice offered through the site. You are responsible for hiring counsel to represent you and your particular interests you wish to protect and promote.

Hornstein Law Offices is owned and operated by Steve Hornstein, Esq. All right, title and interest in and to the information and documents provided on the HornsteinTrust.com site and affiliated sites owned by Hornstein Law Offices, including but not limited to applications, information, documents, logos, graphics, sounds and images (the "Materials"), are owned by Hornstein Law Offices or may be owned by our respective third party authors, developers or vendors ("Third Party Providers"). You are free to print your questionnaire and take the questionnaire to any appropriately licensed attorney you choose to provide you with legal service. You are solely responsible for your estate plan meeting your estate planning objectives. While the HornsteinTrust.com site allows you to have documents prepared in a "Do-It-Yourself" manner without an attorney review or assistance, we recommend that you seek counsel to review your estate plan and/or provide counsel to you to ensure that your plan meets your plan objectives.

The HornsteinTrust.com site is not intended to create any attorney-client relationship, and your use of the site does not and will not create an attorney-client relationship between you and Hornstein Law Offices until and unless you hire Hornstein Law Offices and are accepted as a client of Hornstein Law Offices.

When you use the HornsteinTrust.com site, you will be asked for personal information. You must provide complete and accurate information as requested on the screens. You will also be asked for your email. It is your responsibility to enter your correct email. When you register to use the site, your password will be emailed to the email that you provide. When you register, you should confirm that you have received your password before you continue to enter your personal information on the remainder of the screens on the questionnaire. If you entered an incorrect email, please refrain from using the site and call Hornstein Law Offices at (800) 280-8100 and email Support@HornsteinTrust.com  immediately. You should protect your password carefully. You agree to notify Hornstein Law Offices immediately of any unauthorized use of your account or password. Hornstein Law Offices shall not be liable for any losses you incur as a result of someone else's use of your account or password, either with or without your knowledge. You may be held liable for any losses incurred by Hornstein Law Offices, related companies, representatives, officers, directors, employees, consultants, and agents due to someone else's use of your account or password.

On any such termination, you agree to immediately destroy any downloaded or printed Materials. By hiring Hornstein Law Offices to prepare your documents, whether it be on a self-help or a comprehensive full service basis, you agree that the documents you purchase may only be used by you for your personal or business use or used by you in connection with your client and may not be sold or redistributed without the express written consent of Hornstein Law Offices.
